Job Summary
Provide home-based speech therapy to patients with communication, swallowing, or cognitive difficulties due to illness or injury. Conduct patient assessments, document progress, and ensure goals are met while assisting with mouth, breathing, swallowing exercises, fluency, articulation, and reading. Work as part of a home health team including RNs, LPNs, CNAs, HHAs, PTs, and OTs to support the patient's plan of care. Educate patients and families on therapy goals, safety practices, and self-care. Requires a Master's or Doctoral degree in Speech Language Pathology, active Arizona SLP license, and at least one year of speech therapy experience.
