Job Summary
Conduct home health visits to adult and geriatric patients within designated territories, performing thorough evaluations to assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties. Develop treatment plans based on physician orders and goals, while continually assessing and revising care plans in collaboration with the multidisciplinary team. Educate patients and family members on rehabilitative care to promote independent living, and accurately document all observations, interventions, and evaluations using a touch pad tablet. Requires a current Colorado SLP license and at least one year of experience; pay is $60-65 per point.
Required Qualifications
- At least one year of experience as an Speech Language Pathologist
- A current license as a Speech Language Pathologist in Colorado

- Prior home health experience as an SLP
