Speech Language Pathologist SLP Home Health PRN

Conduct direct Speech-Language pathology services in patient homes, including initial therapy evaluations, re-evaluations, and follow-up assessments per OASIS and PPS guidelines. Develop and revise plans of care based on patient findings, physician orders, and identified care issues while ensuring continuity and proper discharge planning. Instruct patients and caregivers on home therapy programs, select appropriate diagnostic and therapeutic materials, and document all therapy notes and supply logs accurately. Promote agency policies and communicate significant findings or changes in condition to supervisors, physicians, and team members. Requires a Master's degree, current unrestricted license, minimum one year of experience, and willingness to travel 50% in this physically demanding, high-stress environment.
