Job Summary
Conduct home health visits for adult and geriatric clients in New Hampshire, evaluating and treating speech, language, communication, and swallowing challenges. Create personalized care plans, adjust them based on client progress, and partner with other clinicians to deliver comprehensive care. Educate patients and families to ensure confidence in continuing care at home, while documenting all visits and progress using the EMR system. Full-time role with a $15,000 bonus opportunity, offering weekly pay and flexible scheduling.
Required Qualifications
- An active SLP license in New Hampshire
- Graduate of a speech-language pathologist program accredited by the Council on Academic Accreditation in Audiology and Speech-Language Pathology with the American Speech-Language Hearing Association (ASHA)
- Comfort with basic technology and documentation systems
