Job Summary
Speech Language Pathologist responsible for evaluating, assessing, developing, and administering personalized treatment plans for patients with speech and language rehabilitation needs in a home-based setting. Identify communication disorders, modify therapy as needed, and track patient progress through documentation and outcome measures. Build treatment plans and deliver therapy to patients, utilizing evidence-based approaches and maintaining compliance with state licensure requirements and CCC certification. Collaboration with a broader care team and ongoing patient education are emphasized to achieve meaningful communication outcomes in the home environment.
Required Qualifications
- Current licensure or certification required by state regulations
- Successful completion of SLP Certification of Clinical Competence (CCC)
- CPR certification required or must be obtained within 30 days of hire
- Master’s degree preferred, or Bachelor’s degree with field experience
