Job Summary
Establish, assess, and modify realistic, measurable, timely, and functional goals for patients upon physician referral in accordance with Life Care standards. Chart appropriately and utilize therapy software to track progress. Liaise with patients, families, and interdisciplinary teams to plan care, while maintaining confidentiality and adhering to all applicable laws and regulations. Assist in patient evacuation and demonstrate good body mechanics during daily activities. Participate in community health projects as appropriate and maintain current CPR certification.
Required Qualifications
- Graduate of an accredited program in speech therapy (minimum MS or MA SLP)
- ASHA certified (CCCs) OR must be eligible for 9 month Clinical Fellowship Year (CFY) in order to earn ASHA certification (CCCs) and/or meeting ASHA credentialing criteria
- Currently registered/licensed in applicable State
- Must maintain an active license in good standing throughout employment
- CPR certification upon hire or obtain during orientation
- CPR certification must remain current during employment
- Read, write, speak, and understand the English language
