Speech-Language Pathologist (SLP) - Full Time

Job Summary

Conduct comprehensive speech, language, cognitive-communication, and swallowing evaluations for short-term rehab and long-term care residents across three skilled nursing facilities in the Buffalo area. Develop individualized treatment plans with measurable goals, manage dysphagia including diet texture recommendations, and coordinate with physicians, nursing, and dietary staff on care decisions. Complete timely documentation in the facility EMR, screen residents for status changes, and supervise clinical fellows or graduate students as assigned. Maintain compliance with Medicare Part A and B requirements and state regulations while working independently across assigned sites.

Required Qualifications

  • Degree in Speech-Language Pathology from an accredited program
  • Current New York State SLP license (or license-eligible)
  • Valid driver's license
  • Reliable transportation
  • Willingness to travel between the three assigned Buffalo-area sites
  • Ability to work independently
  • Ability to adapt to different facility teams and workflows

Desired Qualifications

  • Prior experience in skilled nursing, subacute rehab, or geriatric care
  • Experience with dysphagia management and instrumental assessment interpretation (MBSS/FEES)
  • Familiarity with PointClickCare or comparable long-term care EMR
  • Working knowledge of MDS, PDPM, and Medicare therapy documentation standards
