Job Summary
Provide home-based speech therapy to patients facing communication, swallowing, or cognitive difficulties due to illness or injury. Observe and evaluate patient abilities, conduct assessments, document progress, and ensure established goals are met. Assist with exercises for mouth, breathing, swallowing, fluency, articulation, and reading. Work within a home health team including RNs, LPNs, CNAs, and therapists to support the patient's plan of care. Educate patients and families on safety practices, self-care, and therapy objectives. Maintain active SLP licensure in California, hold a Master's or Doctoral degree, and possess CPR certification.
Required Qualifications
- Master's or Doctoral degree in Speech-Language Pathology
- Active SLP license in CA
- Minimum of 1 year of speech therapy experience
- CPR certification
- Knowledge of state and federal home health regulations
- Good clinical judgment
