Job Summary

Provide home-based speech therapy to patients facing communication, swallowing, or cognitive difficulties due to illness or injury. Observe and evaluate a patient's ability to communicate, swallow, and process information, then conduct assessments, document progress, and ensure established goals are met. Assist with mouth, breathing, and swallowing exercises, fluency, articulation, and reading while educating patients and families on plan of care, therapy goals, safety practices, and self-care. Work as part of a home health team including RNs, LPNs, CNAs, HHA, PT, and OT, focused on the patient's plan of care and goals. Requires a Master's or Doctoral degree in Speech-Language Pathology with an active CA license, minimum one year of speech therapy experience, and CPR certification. Knowledge of state and federal home health regulations is essential.
