Job Summary
Provide home-based speech therapy to patients facing communication, swallowing, or cognitive difficulties due to illness or injury. Work within a home health team including RNs, LPNs, and OTs to support established patient plans of care. Conduct assessments of communication and swallowing abilities, document progress, and ensure goals are met. Assist with mouth, breathing, and swallowing exercises while educating patients and families on safety practices and self-care. Maintain CPR certification and knowledge of state and federal home health regulations. Master's or Doctoral degree in Speech-Language Pathology with an active CA license and at least one year of experience required.

- Active SLP license in CA
- Minimum of 1 year of speech therapy experience
