Job Summary
Perform comprehensive speech, language, cognitive, and swallowing evaluations, then design individualized treatment plans to promote functional independence. Deliver evidence-based therapy for communication, cognition, and dysphagia while monitoring resident progress and updating plans as needed. Collaborate with physicians, nursing staff, PTs, OTs, and families to provide coordinated care, maintaining timely, accurate documentation in compliance with regulatory requirements. Work in a collaborative skilled nursing and post-acute care environment treating a diverse caseload of short-term rehabilitation and long-term care residents.
Required Qualifications
- Active Illinois Speech-Language Pathologist license
- Excellent communication skills
- Clinical decision-making skills
