Job Summary
Speech Language Pathologist (SLP) provides direct rehabilitation in speech therapy to patients in a Skilled Nursing Facility setting. Requires MS or MA in Speech-Language Pathology, ASHA certification (CCC) or CFY eligibility, and an active State license. Must maintain credentialing and CPR certification; preferred 1 year of experience. Responsibilities include establishing and modifying measurable goals, charting, coordination with an interdisciplinary team, and delivering patient-centered care in post-acute and long-term rehab contexts. Essential functions involve implementing therapy plans, documenting progress, ensuring patient rights and safety, and utilizing basic software (Word, Excel) to support care delivery.
