Responsibilities include evaluating patients, developing and implementing therapy goals and treatment plans, coaching caregivers to carry over activities, coordinating with referral partners, ensuring continuation of the therapeutic plan after discharge through home exercise programs, and documenting care in an electronic medical record while maintaining patient confidentiality. Qualifications include ASHA certification, a state license, and current CPR certification; at least 1 year of experience is preferred. The role supports both part-time and full-time schedules with a guaranteed pay option to build caseload.
