Job Summary
Provide home-based skilled speech therapy care for patients in the Dayton, Trotwood, and West Carrollton area as directed by the attending physician. Develop an individualized plan of care for Speech Therapist (ST), educate patients and families, document according to agency/Medicare guidelines, and collaborate with an interdisciplinary team to improve swallowing, cognition, speech, and communication while minimizing risk. Must maintain licensure, meet minimum experience requirements, and be able to travel to patient homes with a valid driver's license. Our role emphasizes compassionate, patient-centered care and adherence to agency policies and Medicare guidelines.
Required Qualifications
- Speech Therapist license in the state of employment
- Minimum one-year experience as a Speech Therapist (ST) in an acute care setting
- Valid driver's license and auto insurance in your name as a driver
