Job Summary
Conduct one-on-one Speech Language Pathology evaluations, develop individualized plans of care, and deliver evidence-based treatment to older adults in private residences, senior living communities, or via telehealth. Provide therapy sessions supporting patient mobility, independence, and functional longevity while documenting progress within FOX's EMR system. Collaborate with interdisciplinary teams including PTs, OTs, and Fitness Specialists to educate patients and caregivers on safety and aging in place. Manage your own treatment schedule with flexibility to incorporate virtual care components as needed.
Required Qualifications
- Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Speech Language Pathology program
- Basic computer and EMR proficiency
- Current CPR certification
- Current Certificate of Clinical Competence (ASHA CCC)
- Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Performing CPR
- Providing moderate to maximum assistance with patient transfers and bed mobility
- Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
- Driving to patient homes and senior living communities as part of the Geriatric House CallsTM model
- Wearing PPE when required
