Job Summary
Conduct one-on-one Speech Language Pathology to older adults in private residences, senior living communities, and memory care settings. Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment supporting patient mobility, independence, and functional longevity. Document sessions effectively within FOX's EMR system and collaborate with interdisciplinary teams including PTs, OTs, and Fitness Specialists. Educate patients, caregivers, and staff to promote safety and aging in place. Provide telehealth or virtual care when appropriate to extend access and flexibility. Manage your own treatment schedule while meeting physical demands such as patient transfers, lifting, and driving to sites.
Required Qualifications
- Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Speech Language Pathology program
- Basic computer and EMR proficiency
- Current CPR certification
- Current Certificate of Clinical Competence (ASHA CCC)
- Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Performing CPR
- Providing moderate to maximum assistance with patient transfers and bed mobility
- Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
- Driving to patient homes and senior living communities as part of the Geriatric House CallsTM model
- Wearing PPE when required
