Job Summary
Provide one-on-one Speech Language Pathology to older adults in homes or Senior Living Communities (private residences, memory care, independent living, assisted living) under the Geriatric House Calls model; perform evaluations, develop individualized plans of care, and deliver evidence-based treatment; deliver therapy sessions to support patient wellness, mobility, and functional independence; document in FOX’s EMR; collaborate with an interdisciplinary team; educate patients, caregivers, and staff; provide telehealth/virtual care when appropriate to extend access; opportunities to work in-home and senior living settings with flexible scheduling and clinician-driven growth.
Required Qualifications
- Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Speech Language Pathology program
- Basic computer and EMR proficiency
- Current CPR certification
