Speech Language Pathologist

Direct all aspects of quality patient care, including evaluation, treatment planning, goal setting, and documentation according to company guidelines and professional standards. Provide professional treatment using therapeutic techniques for rehabilitation of patients with speech, language, hearing, oral motor, swallowing, and cognitive disorders. Perform patient evaluations, administer standardized assessments, conduct swallowing evaluations, and develop treatment plans consistent with physician orders. Carry out patient care plans, determine appropriate progression, and obtain approval for services from referring physicians. Document evaluations, progress notes, and discharge summaries in medical records as defined by department policy. Participate in patient care planning and utilization review meetings while assisting speech therapy assistants. Demonstrate knowledge and skills necessary to provide patient care appropriate in a SNF setting.
