Job Summary
Conduct comprehensive assessments and evaluations using subjective and objective measures to develop treatment plans and programs in accordance with physician orders. Provide quality treatments that actively progress goals to achieve the highest quality of life for residents and clients. Perform screenings and educate residents, families, caregivers, and staff on relevant services. Coordinate seamless discharges utilizing the full continuum of therapy and fitness services while providing accurate documentation and billing in electronic medical records. Demonstrate knowledge of payer sources and regulatory requirements, and complete supervisory visits per regulations. Communicate effectively with interdisciplinary teams, physicians, and referral sources.
Required Qualifications
- CCC-SLP certification
- Part Time experienced Speech Language Pathologist
- Graduate from an accredited Speech Language Pathology program
- Current licensure in the state(s) of practice as required
- Demonstrated ability to read, write, speak and understand the English language to communicate with all customers
