Job Summary

Evaluate residents for communication problems and develop individualized speech-language therapy treatment plans. Conduct audiometric screenings, coordinate referrals for audiology services, and initiate therapy programs based on physician orders. Collaborate with PTs, OTs, nursing staff, and physicians to optimize resident outcomes while educating families on treatment strategies. Maintain accurate documentation, including progress notes, discharge summaries, and statistical reports for Medicare billing. Confers with other staff to integrate speech-language goals with interdisciplinary approaches and attends care planning meetings. Participate in developing in-service training programs and review facility policies annually. Submit reports to the Business Office and complete assessment forms as requested.

Required Qualifications

  • Master's Degree in Speech-Language Pathology
  • Current New York State SLP license
  • Ability to communicate well verbally and in writing
  • Current, valid CPR/BLS credential
  • Qualities of maturity, diplomacy, and ability to work well with others
  • Demonstrated willingness to uphold the vision and mission of the company by following the core values of Integrity, Team work, Passion for Excellence and Reverence for the Individual
  • Ability to read, write, speak, and understand the English language at an intermediate or more advanced level
  • M.A. or M.S. Degree in Speech-Language Pathology from an approved school
  • State license to practice as a Speech-Language Pathologist
  • Clinical Competence Certification from the American Speech and Hearing Association
  • Skilled Nursing Facility (SNF) or geriatric experience
  • Strong communication and teamwork skills
  • Passion for helping residents regain function and quality of life
  • Ability to perform tasks to established standards of excellence
  • Verification of qualifications by the State Department of Health
  • Rehabilitation Therapies (license number/certification number)

Desired Qualifications

  • Certificate of Clinical Competence (CCC-SLP) preferred

Additional Requirements

  • Employee must be able to read, write, speak, and understand the English language at an intermediate level or more advanced
