Job Summary
The Speech/Language Pathologist evaluates, develops, implements and documents an intervention plan across multiple service sites, performing evaluations, providing intervention and discharge planning, ensuring documentation meets third-party payer requirements, and collaborating with therapy team members to achieve patient goals. The role requires coordination across sites, ongoing documentation, and relationship-building with facility staff, with responsibilities including progress documentation, discharge planning, follow-up resources, and adherence to operating procedures.
Required Qualifications
- Masters in Speech Pathology
- CCC-SLP (Certificate of Clinical Competence in Speech-Language Pathology)
- ASHA
- ND State licensure or eligibility to practice Speech-Language Pathology
- Minnesota Department of Health licensure in Audiology or equivalent (as applicable)
- BLS within required timeframe
