Job Summary
Provide and manage Speech-Language Pathology services in a long-term care facility, including screening, evaluations, and development and modification of individual treatment plans; document progress, discharge residents when appropriate, and collaborate with the interdisciplinary team to ensure compliance with Medicare/Medicaid policies and facility procedures. Maintain confidentiality, adhere to policies, utilize electronic clinical software, and participate in discharge planning and care coordination with ongoing professional growth.
Required Qualifications
- Graduate of an accredited school of Speech-language Pathology with a Master’s Degree or higher
- Valid NYS license as a Speech-Language Pathologist
- Valid ASHA certification in Speech-Language Pathology
- Minimum of 1-year experience in a long-term care facility
