Job Summary
Provide one-on-one Speech Language Pathology to older adults in private residences, senior living communities, and via telehealth. Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment supporting patient mobility, independence, and functional longevity. Document sessions within FOX's EMR system and collaborate with interdisciplinary teams including PTs, OTs, and Fitness Specialists. Educate patients, caregivers, and staff to promote safety and aging in place. Create and manage your own treatment schedule while maintaining flexibility to incorporate virtual care components.
Required Qualifications
- Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Speech Language Pathology program
- Basic computer and EMR proficiency
- Current CPR certification
- Current Certificate of Clinical Competence (ASHA CCC)
- Ability to exert medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Ability to perform CPR
- Ability to provide moderate to maximum assistance with patient transfers and bed mobility
- Ability to reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities
- Ability to drive to patient homes and senior living communities as part of the Geriatric House CallsTM model
- Ability to wear PPE when required
