Job Summary

Assess and evaluate therapeutic, rehabilitative, and functional status to identify speech defects and recommend communication devices. Direct the speech language pathology treatment plan, re-evaluating progress and revising the plan as necessary. Collaborate with physicians and team members to develop, implement, and support the patient's plan of care while instructing other personnel in specific phases. Maintain accurate clinical notes, submit weekly documentation per agency guidelines, and report changes in patient condition to supervisory personnel. Provide services in private homes, traveling frequently to complete visits, and assist caregivers and families in ensuring continuity of care. Adhere to MGA policies, dress code, and professional appearance standards while participating in mandated in-services.

Required Qualifications

  • Master's degree in Speech Language Pathology
  • Current License and in good standing with State Licensing Board
  • Has Certificate of Clinical Competence in Speech Language Pathology by ASHA
  • Successful completion of appropriate screening test with passing rate
  • Current CPR certification per state regulations
  • Professional experience required per state regulations
  • Displays good observation skills and judgement as well as good oral and written communication skills
  • Reliable Transportation
  • Ability to regularly sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull and reach with hands and arms and other physical activities as deemed necessary by job duties
  • Vision and Hearing capabilities within normal limits, with or without aid
  • Ability to frequently lift and or move up to 15 pounds and occasionally lift or move 100 pound or more if applicable
  • Ability to perform the anticipated level of physical assist required to provide safe client care
  • The employee is occasionally exposed to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals
  • The employee is required to travel frequently to complete home health visits
