Job Summary
Provide one-on-one Speech Language Pathology to older adults wherever they call home (private residences, senior living communities such as independent living, assisted living, and memory care). Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment to support wellness, mobility, and functional independence. Deliver therapy sessions with documentation in FOX’s EMR, collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists), and educate patients, caregivers, and staff to promote safety and aging in place. May incorporate telehealth/virtual care to extend access and flexibility across in-home and senior living settings. The role offers autonomy in scheduling, opportunities for in-home and community-based care, and a clinician-driven culture with pathways for professional growth. Required Qualifications
- Valid Speech Language Pathology license in the state(s) of practice
- Degree from an accredited Speech Language Pathology program
- Current CPR certification
- Current Certificate of Clinical Competence (ASHA CCC)
