Speech Language Pathologist

Assess, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders for patients across diverse age groups. Develop individualized treatment plans, collaborate with an interdisciplinary care team, maintain accurate clinical documentation, and provide education to patients and families. This role supports the Healthcare & Medical Services team at Heritage Health Care Center, part of HHP Lifeshare, within a collaborative environment focused on meaningful, patient-centered care and improved health outcomes.
