Job Summary
Assess swallowing and communication, then plan, implement, and monitor interventions for diverse patient populations across acute care and transitional care units. Provide education and counselling to patients, families, and interprofessional team members regarding safe swallowing and communication strategies. Manage complex discharges within the transitional care program while maintaining patient records and workload measurement per facility standards. Act as an SLP resource to the team, families, and students, and participate in interprofessional education activities and hospital-wide committees. Supervise one student placement annually and contribute to ongoing teaching and knowledge dissemination in support of the University of Toronto affiliation. Work three days per week as part of the Priority Coverage Team and two days at the Holland Centre, adhering to College of Audiologists and Speech-Language Pathologists of Ontario standards.
Required Qualifications
- Current CASLPO registration
- Experience with videofluoroscopic evaluation of swallowing
- Experience providing patient/family education
- Practice in accordance with the College of Audiologists and Speech-Language Pathologists of Ontario
Desired Qualifications
- Minimum of 2 years experience in a hospital setting, preferred
- Previous experience working with medically complex patients in both acute care and transitional care
- Experience with dysphagia and communication assessment and management
- Demonstrated commitment to student education
