Speech Language Pathologist

Conduct evaluations, assess home environments, and identify equipment needs relative to speech/language function. Identify functional speech defects and establish plans of care to improve patient function, implementing revisions as necessary. Provide education and instruction to patients, family members, caregivers, and the interdisciplinary team on care recommendations. Prepare and submit notes per agency policy while performing other duties to facilitate service delivery. This role supports communication, swallowing, and cognitive disorder treatment at Westridge Health Care Center in Terre Haute, Indiana.
