Job Summary
Evaluate, direct, and provide speech/language pathology services to patients in homes or facilities. Administer diagnostic tests including audio logic screening and interpret results. Develop, review, and maintain Plans of Treatment and Care. Observe, record, and report patient condition changes and treatment responses to supervisors and physicians. Provide instruction on alternative communication systems and counsel patients, families, and healthcare staff. Submit documentation, evaluations, visit summaries, care plans, and progress reports per company policy. Participate in care coordination and discharge planning. Maintain confidentiality and professional conduct. Requires valid driver's license, auto insurance, and reliable transportation. Pay is per visit/unit rate.
Required Qualifications
- Minimum six months experience as a speech therapist / speech language pathologist
- Home Health experience
- A valid driver's license
- Auto insurance
- Reliable transportation
