Speech Language Pathologist - HomeCare Health Full-Time

Evaluate, direct, and provide speech/language pathology services to patients in homes or facilities. Administer diagnostic tests, interpret results, and treat disorders of speech, voice, language, hearing, and swallowing. Develop, review, and submit Plans of Treatment and Care while documenting evaluations, visits, and progress reports. Provide instruction to patients and families on alternative communication systems and counsel healthcare staff. Observe and report patient condition changes to supervisors and physicians. Participate in care coordination and discharge planning. Maintain strict confidentiality and professional conduct. Requires minimum six months speech therapy experience, valid driver's license, auto insurance, and reliable transportation. Pay per visit/unit rate.
