Job Summary
Evaluate, direct, and provide speech/language pathology services to patients in homes or facilities, focusing on rehabilitation and functional improvement. Administer diagnostic tests including audio logic screening and interpret results to assess disorders of speech, voice, language, hearing, and swallowing. Develop, review, and update Plans of Treatment and Care while observing and reporting patient condition changes to supervisors and physicians. Provide instruction on alternative communication systems and counsel patients, families, and healthcare staff. Maintain documentation, submit written reports, and participate in care coordination and discharge planning. Requires a valid driver's license, auto insurance, and reliable transportation.
Required Qualifications
- Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
- Minimum six months experience as a speech therapist / speech language pathologist
- A valid driver's license
- Auto insurance
- Reliable transportation
