Job Summary
Evaluate, direct and provide speech/language pathology services to patients in the home or facility. Participate in the development and periodic review of the Plan of Treatment and Plan of Care. Administer and interpret diagnostic tests and applications of therapeutic treatments for disorders of speech, voice, language, hearing and swallowing. Observe, record and report changes in patient condition to supervisor/physician; provide instruction in alternative communication systems; counsel patients, families and healthcare staff; maintain and submit documentation and progress reports; participate in care coordination and discharge planning. Requires minimum six months of Home Health experience, valid driver's license, auto insurance, and reliable transportation. Location: Tacoma, WA; environment: home health visits; full-time role with 40 hours/week.
