Speech Language Pathologist Home Health

Conduct comprehensive speech, language, cognitive, communication, and swallowing evaluations while developing and implementing individualized plans of care based on patient needs and physician orders. Provide skilled treatment to improve communication, cognitive-linguistic function, and swallowing abilities, educating patients, families, and caregivers on treatment plans, techniques, and safety precautions. Monitor patient progress and modify treatment plans as clinically appropriate, communicating and collaborating with physicians, nurses, therapists, and other members of the interdisciplinary team. Complete accurate and timely clinical documentation in accordance with company, Medicare, and regulatory requirements, participating in care coordination and discharge planning as needed while maintaining patient confidentiality. This field-based per diem position offers flexible scheduling and competitive per-visit rates for licensed SLPs in Broward and Palm Beach Counties, Florida.
