Job Summary
Conduct comprehensive evaluations and implement individualized plans of care for patients with speech, language, cognitive, communication, and swallowing disorders. Provide skilled treatment to improve functional abilities while educating families and caregivers on safety and techniques. Monitor progress, modify treatment plans, and coordinate discharge planning with physicians and the interdisciplinary team. Complete accurate clinical documentation in accordance with Medicare and regulatory requirements. Maintain patient confidentiality and adhere to HIPAA standards. This field-based role offers flexible scheduling and per-visit rates for licensed SLPs in Broward and Palm Beach Counties.
Required Qualifications
- Master's degree in Speech-Language Pathology from an accredited program
- Current and unrestricted Florida Speech-Language Pathologist license
- Current CPR/BLS certification
- Reliable transportation
- Willingness to travel to patients' homes within the assigned service area
- Ability to work independently
- Ability to effectively collaborate with the care team
Desired Qualifications
- Certificate of Clinical Competence (CCC-SLP)
- Home health experience
- Experience working with adult and/or geriatric patients
