Job Summary
Conduct comprehensive speech, language, cognitive, communication, and swallowing evaluations for patients in their homes. Develop and implement individualized plans of care based on patient needs and physician orders, while providing skilled treatment to improve communication, cognitive-linguistic function, and swallowing abilities. Educate patients, families, and caregivers on treatment plans, techniques, and safety precautions. Monitor patient progress and modify treatment plans as clinically appropriate, communicating and collaborating with physicians, nurses, therapists, and other members of the interdisciplinary team. Complete accurate and timely clinical documentation in accordance with company, Medicare, and regulatory requirements. Participate in care coordination and discharge planning as needed, maintaining patient confidentiality and adhering to HIPAA requirements.
Required Qualifications
- Master's degree in Speech-Language Pathology from an accredited program
- Current and unrestricted Florida Speech-Language Pathologist license
- Current CPR/BLS certification
- Reliable transportation
- Willingness to travel to patients' homes within the assigned service area
- Ability to work independently
- Ability to effectively collaborate with the care team
Desired Qualifications
- Certificate of Clinical Competence (CCC-SLP)
- Home health experience
- Experience working with adult and/or geriatric patients
