Speech Language Pathologist-Home Health Visits

Job Summary
Conduct home visits to clients in designated Columbia County territories to assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties. Develop and continuously revise treatment plans based on evaluations, physician orders, and recommended goals while collaborating with a multidisciplinary care team. Educate patients, family members, and patient representatives on rehabilitative care and activities to promote independent living. Accurately document all observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Requires a current Pennsylvania Speech Language Pathologist license and solid computer skills; prior home care experience is a plus.
Required Qualifications
- A current license as a Speech Language Pathologist in Pennsylvania
- Possess a certificate of clinical competence in speech from the American Speech and Hearing Association
- Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification
- Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification
Desired Qualifications
- Prior home care experience as a Speech Language Pathologist, SLP
- Solid computer skills
- Prior experience with electronic medical records (EMR)
