Speech Language Pathologist, Home Health Visits

Job Summary
Conduct home visits to clients in designated geographic territories to perform thorough evaluations, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties. Develop treatment plans based on physician orders and goals, then continually assess and revise care plans while participating in the multidisciplinary team. Educate patients and family members on rehabilitative activities to promote independent living, and accurately document all observations and interventions using a touch pad tablet. Requires a current Pennsylvania SLP license and solid computer skills; offers part-time or per diem opportunities with comprehensive benefits and training opportunities.
Required Qualifications
- A current license as a Speech Language Pathologist in Pennsylvania
- Possess a certificate of clinical competence in speech from the American Speech and Hearing Association
- Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification
- Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification
Desired Qualifications
- Prior home care experience as a Speech Language Pathologist, SLP
- Solid computer skills
- Prior experience with electronic medical records (EMR)
