Speech Language Pathologist, Home Health SLP - Part time

Job Summary
Develop speech therapy plans of care for patients with speech and language complications following physician consultation. Conduct home visits for assessment, evaluation, and OASIS documentation while communicating with physicians and interdisciplinary teams. Provide education and instruction to patients, families, and staff on communication, swallowing exercises, and psychosocial aspects. Supervise home health aides as needed and participate in quality improvement programs and regularly scheduled team meetings. Evaluate treatment outcomes, recommend alternative communication mechanisms, and plan appropriate discharges. Maintain accurate medical records, billing documentation, and EMR entries in compliance with standard precautions and infection control protocols.
Required Qualifications
- Bachelor's degree in Speech Language Pathology
- Minimum of one (1) year of experience in a home health setting
- Active and unencumbered Speech Language Pathologist license in state(s) of employment
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
- Ability to compute rate, ratio, and percentage
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to write reports, business correspondence, and procedure manuals
- Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
- Strong written and verbal communications
- Ability to understand, read, write, and speak English
- Regular standing, walking, and manual dexterity
- Ability to lift and move objects up to 50 pounds
- Close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
- Routine use of standard medical equipment and tools associated with clinical care
