Job Summary
PRN Speech Language Pathologist in home health providing assessment and treatment of speech, language, cognitive, and swallowing disorders. Responsible for initial and ongoing assessments (OASIS/Agency), developing and revising the Plan of Care in collaboration with the physician and care team, ensuring quality continuity of care, and maintaining appropriate documentation. Requires a Bachelor's degree in Speech Language Pathology (Master's preferred), active SLP license, and at least one year of home health experience. Provides services with family involvement and coordinates with nurses and other health care professionals to optimize patient outcomes.
Required Qualifications
- Bachelor's degree in Speech Language Pathology
- Active and unencumbered Speech Language Pathologist license in state(s) of employment
- Minimum of one year of experience in a home health setting
