Job Summary
Speech Language Pathologist (Home Health) provides speech, language, cognitive, and swallowing services in patients' homes under physician direction; conducts comprehensive assessments (OASIS/Agency), develops/implements revisions to the Plan of Care with the care team, documents outcomes, and ensures continuity of quality patient care. Responsibilities include infection control, patient/family education, coordination with interdisciplinary team, supervision of Home Health Aide/UP, required regulatory compliance, and ongoing professional development. Requires a Bachelor's in Speech Language Pathology (Master's preferred), active SLP license, and at least one year of home health experience.
Required Qualifications
- Bachelor's degree in Speech Language Pathology
- Active and unencumbered Speech Language Pathologist license in state(s) of employment
- Minimum of one year of experience in a home health setting preferred (required by description)
- Master's degree in Speech Language Pathology preferred
