Job Summary
Speech Language Pathologist (SLP/CCC) for Home Health and Community-Based care. Part-Time and PRN opportunities serving patients in home health, assisted living, independent living, and personal care settings across Philadelphia County, Montgomery County, and Bucks County. Responsibilities include evaluating speech, language, cognitive, and swallowing disorders; developing and implementing individualized treatment plans; delivering in-home and community-based speech therapy; performing reassessments, progress notes, and discharge planning; educating patients, families, and caregivers; collaborating with physicians and interdisciplinary teams; completing documentation in HCHB EMR; and managing caseload and schedule. Ideal candidates have Home Health experience and can work independently with a patient-centered approach.
Required Qualifications
- Master’s or Doctorate in Speech-Language Pathology
- Active Pennsylvania SLP License
- CCC-SLP (ASHA) – Required
- Home Health experience – Required
- Experience with Home Care Home Base (HCHB) EMR – preferred
- Valid driver’s license & reliable transportation
- Strong communication, organization, and time management skills
