Job Summary

Make home visits to clients in designated geographic territories to perform thorough evaluations for speech, language, communication, or swallowing difficulties. Develop treatment plans based on physician orders and goals, while continually assessing care plans and participating in multidisciplinary discussions. Educate patients and family members on rehabilitative activities to promote independent living, and accurately document all observations and interventions using a touch pad tablet. This role supports adult and geriatric patients in homes throughout Washington and Orange County, Vermont, as part of a multi-disciplinary team providing skilled nursing and rehabilitative care.

Required Qualifications

  • A current license as a Speech Language Pathologist in Vermont
  • Possess a certificate of clinical competence in speech from the American Speech and Hearing Association
  • Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification
  • Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification

Desired Qualifications

  • Solid computer skills
  • Prior experience with electronic medical records (EMR)
