Job Summary

Conduct home visits to clients in designated geographic territories to perform thorough evaluations, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties. Develop treatment plans based on physician orders and goals, then continually assess and revise care plans while participating in multidisciplinary team discussions. Educate patients and family members on rehabilitative care and independent living activities, and accurately document all observations, interventions, and evaluations using a touch pad tablet. Requires a current Vermont SLP license and solid computer skills; base pay ranges from $46.00 to $50.00 per hour.

Required Qualifications

  • A current license as a Speech Language Pathologist in Vermont
  • Possess a certificate of clinical competence in speech from the American Speech and Hearing Association
  • Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification
  • Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification

Desired Qualifications

  • Prior home care experience as a Speech Language Pathologist, SLP
  • Solid computer skills
  • Prior experience with electronic medical records (EMR)
