Job Summary
Conduct home visits to clients in designated territories to perform evaluations, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties. Develop treatment plans based on physician orders and goals, while continually assessing and revising care plans within the multidisciplinary team. Educate patients and family members on rehabilitative activities to promote independent living, and accurately document all observations and interventions using a touch pad tablet. Requires a current Indiana Speech Language Pathologist license and solid computer skills; per diem opportunity in Indianapolis, IN.
Required Qualifications
- A current license as a Speech Language Pathologist in Indiana
- Possess a certificate of clinical competence in speech from the American Speech and Hearing Association
- Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification
- Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification
Desired Qualifications
- Prior home care experience as a Speech Language Pathologist, SLP
- Solid computer skills
- Prior experience with electronic medical records (EMR)
