Job Summary
Practice independently in the community as part of an interdisciplinary care team, conducting comprehensive assessments of patients to determine needs, developing action plans including treatment plans and outcomes, and delivering personalized rehabilitation therapy to patients in their home or care facilities. Continually evaluate evolving patient needs and adjust plans of care accordingly. Requires NY State licensure for Speech Language Pathologist/Audiologist, at least 1 year of experience or completion of probationary period as a VNS Health Rehab Intern, and a valid driver’s license for areas outside Manhattan; per diem role with daytime Monday–Friday availability.
Required Qualifications
- Minimum of one-year speech language pathology experience or successful completion of probationary period as a VNS Health Rehab Intern
- Licensure to practice as a Speech Language Pathologist/Audiologist in New York State
- Valid driver's license required for all areas outside the borough of Manhattan
