Job Summary
Provide one-on-one Speech Language Pathology to older adults in private residences, senior living communities, and memory care settings. Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment supporting patient wellness, mobility, and functional independence. Document effectively within FOX's EMR system and collaborate with an interdisciplinary team of PTs, OTs, and Fitness Specialists. Educate patients, caregivers, and staff to promote safety and aging in place, while incorporating telehealth or virtual care when appropriate to extend access and flexibility. Create and manage your own treatment schedule within a clinician-operated practice rooted in meaningful relationships and a mission to abolish ageism.
Required Qualifications
- Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Speech Language Pathology program
- Basic computer and EMR proficiency
- Current CPR certification
- Current Certificate of Clinical Competence (ASHA CCC)
- Ability to exert medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Ability to perform CPR
- Ability to provide moderate to maximum assistance with patient transfers and bed mobility
- Ability to reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities
- Ability to drive to patient homes and senior living communities as part of the Geriatric House CallsTM model
- Ability to wear PPE when required
- Must be available for weekend shifts
- Must be available for overtime
- Must be available for travel
