Job Summary
Provide one-on-one Speech Language Pathology to older adults in private residences and senior living communities. Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment sessions supporting mobility and functional independence. Document effectively within FOX's EMR system and collaborate with an interdisciplinary team of PTs, OTs, and Fitness Specialists. Educate patients, caregivers, and staff to promote safety and aging in place. Incorporate telehealth or virtual care when appropriate to extend access and flexibility. Manage your own treatment schedule while driving to patient locations as part of the Geriatric House Calls model.
Required Qualifications
- Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Speech Language Pathology program
- Basic computer and EMR proficiency
- Current CPR certification
- Current Certificate of Clinical Competence (ASHA CCC)
- Ability to exert medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
- Ability to perform CPR
- Ability to provide moderate to maximum assistance with patient transfers and bed mobility
- Ability to reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities
- Ability to drive to patient homes and senior living communities as part of the Geriatric House CallsTM model
- Ability to wear PPE when required
- Must be cleared through the Florida Background Check Clearinghouse
