Job Summary
Conduct evaluation and treatment of patients with speech, language, hearing, oral motor, swallowing, and cognitive deficits. Develop treatment plans with short- and long-term goals, administer standardized assessments, and monitor progress. Direct patient care including goal setting, family education, and documentation of evaluations, progress notes, and discharge summaries. Collaborate with occupational therapists, physical therapists, physicians, and other healthcare team members to coordinate individualized treatment programs. Ensure delivery of quality rehabilitation services in skilled nursing facilities while maintaining compliance with company guidelines and professional standards.
Required Qualifications
- Speech Language Pathologist CF or CCC
- Direct all aspects of quality patient care
- Provide professional treatment for speech, language, hearing, oral motor, swallowing, and cognitive disorders
- Plan activities and provide therapeutic services for speech and language disorders
- Perform patient evaluations and develop treatment plans with short-term and long-term goals
- Demonstrate knowledge and skills of administering standardized speech/language assessments
- Conduct swallowing evaluations
- Develop treatment plans and monitor patient progress
- Collaborate with occupational therapist, physical therapist, physicians, and other healthcare team members
- Document patient evaluations, treatment progress notes, and discharge summaries
- Prepare and complete a Plan of Care, progress notes, phone orders, and other clinical record documentation
- Ensure delivery of quality rehab services in the SNF
- Demonstrate courtesy, compassion, and respect to patients, caregivers, physicians, and co-workers
- Maintain compliance with Compliance Program and adhere to the Company's Code of Conduct
- Work with the facility and organizations on case management needs and requirements
