Speech Language Pathologist

Conduct in-home evaluations and treatment for adult patients with communication and swallowing disorders. Assess patient needs, develop and implement individualized care plans, and collaborate with an interdisciplinary team to optimize outcomes. Educate patients and families, document progress, and ensure continuity of care within CenterWell's compassionate, integrity-driven culture focused on evidence-based practice and patient-centered home health services.
