Job Summary
Evaluate and treat patients with speech, language, swallowing, and cognitive deficits in a skilled nursing facility setting. Direct all aspects of patient care from evaluation through treatment planning and progress documentation, focusing on improving intelligibility, communication, and swallowing safety. Develop and implement evidence-based treatment plans, collaborate with occupational and physical therapists and physicians, and ensure timely documentation of progress notes and discharge summaries. Required Qualifications
- Licensed Speech Language Pathologist with CCC-SLP (ASHA)
- Master's degree in Speech-Language Pathology

- Experience in SNF/rehabilitation settings preferred
- Documentation and interdisciplinary collaboration skills
